Full time employees have the opportunity to enroll in either a single or family University sponsored group medical insurance plan. The University sponsored group medical insurance plans will be a comprehensive medical plan that covers all hospital services and physician's fees subject to deductibles and out of network copays, on a "usual, customary and reasonable" basis. The Medical Insurance plan will provide a level of coverage as outlined in the present Health Care Plan.
Prescription drugs including mail order option, are included under the deductible amount. The employee has the option to choose the plan during the open enrollment period once each year or on initial employment.
Vision coverage provided will be comparable to the current plan that provides benefits for: an annual eye exam, annual lens replacement (or contact lens instead of glasses), and biannual frame replacement subject to applicable deductibles, percentage limitations and other terms and conditions set forth in the contract with the Univesity's insurance provider.
Dental coverage provided will be comparable to the current plan that provides benefits for preventative, restorative, and orthodontia services subject to applicable deductibles, percentage limitations and other terms and conditions set forth in the contract with the University's insurance provider. Presently, the coverage set forth covers: Group I (Non-orthodontic preventive) paid at 100% with no deductible; Group II (on-orthodontic basic dental services) paid at a rate of 80%; Group III (Non-orthodontic major dental services) paid at a rate of 50%; Group IV (Orthodontic services) paid at a rate of 50% to a maximum benefit of $1000 per insured individual.
Life Insurance and Accidental Death and Dismemberment
The University provides a Life Insurance and Accidental Death and Dismemberment policy for all full-time employees. The plan is based on a death benefit of 100% of designated benefits to employees under sixty-five (65) years of age. The coverage is subject to the limitations stated in the policy.
Long-Term Disability Insurance
The University provides full-time employees with monthly income benefit insurance in cases of long-term disability. The coverage is effective one month after employment and is based on 70% of the monthly earnings or $5,000 per month, whichever is less, and has a waiting period of ninety (90) days and other limitations as stated in the policy.
