Services, or better termed as Accommodations, vary as to the needs of the student.
Accommodations
Our mission "is not to make things easier, but to make things equal". Accommodations are recommended after eligibility has been established based on student provided documentation of the disability. The student is considered the responsible party and would be expected to self proclaim by contacting the Accessibility Office. Accommodations are the result of a signature agreement of Student and Counselor. Each Semester would have a new course schedule, different instructors, and possibly different accommodations. The enrolled student is expected to provide a current schedule to renew his/her accommodations each semester.

To obtain the required Intake, and consent forms.
I need to explain that the lower portion of the Intake form is used to update documentaion The update is important so that we can make sure that you get the right start, and in the right classes. All you need to do is to mark the line in front of the question that relates to your situation.
The Consent to Release form is to protect your rights under the ADA law that effects post secondary education. Please read carefully before you sign it. Please designate by writing in blanks with whom I can communicate. The consent form will require a signature to be activated.
Please click "Grievance" to print and then sign as being aware of this informative document. The Accessibility Office will copy the signature for file, and return the original document for your use.
